Here is the form they will need to fill out.  In addition, I will need
their original receipts and a copy of their passport and visa if they
are not citizens.  If they drove to UCSD, I will need their car
license number, make and model, and the number of roundtrip miles.  I
will make up travel reimbursement packets for these people with
return-addressed envelopes to send to me and include my card if they
have questions.
